Your Hospice Team
Nurses: Your nurses will help coordinate your care plan, pain, medication and symptom management, and collaborate with the rest of your team to provide the comfort and compassion you need.
Home Health Aides: Your hospice aide will help with your daily routine and hygiene, and upkeep with the daily tasks needed to ensure your proper care.
Social Workers: Your social workers provide counseling and support to you and your family in this difficult time.
Chaplains: Offer counseling and spiritual support when needed.
Volunteers: Your volunteer will provide companionship and relief.
Medical Director: Your hospice Medical Director works closely with your physician to provide oversight of your care plan.
