Medical Care

Harry Hynes Memorial Hospice provides physician-directed care, coordinated by a registered nurse. The medical director and registered nurses work closely with the patient's physician to ensure that the patient is kept as physically comfortable as possible. Considerable focus is given to pain control and symptom management to meet care plan goals. The hospice nurse makes scheduled patient visits, and provides instructions on patient care to family members and caregivers. In addition, nursing staff are on call 24 hours a day, seven days a week for emergency situations.

Home Health Aide Services

Harry Hynes Memorial Hospice health care aides assist the family with the personal care of the patient, such as baths, shampoos and nail care. The health care aide also helps with light household duties related to the care of the patient.

Support Services

Harry Hynes Memorial Hospice clinical social workers provide individual and family counseling to help the patient and family adjust to a terminal illness and loss. They also help patients and families access other resources such as Meals on Wheels, Social Rehabilitation Services, Stay Alone Program, Senior Services, Department on Aging, etc. to provide care and enable patients to stay in their place of residence. Social workers are trained to work with both children and adult family members to help them understand and deal with loss issues.

Pastoral Counseling

Harry Hynes Memorial Hospice provides spiritual support to patients and families who desire it. Spiritual issues are an important aspect of dealing with death, both for patients and their family members. The role of the Harry Hynes Memorial Hospice chaplain is to assist the patient and family in using their own faith as a means of coping with a terminal condition. Pastoral counseling also provides a resource for support and education to clergy.  For additional information, please email Tom Welk.

Bereavement Support

Following the patient's death, Harry Hynes Memorial Hospice provides services to help loved ones cope with their loss. A licensed mental health professional directs a comprehensive, 14 month bereavement program that may include individual and family counseling, grief education, a monthly bereavement newsletter, adult and child support groups, seminars, and memorial services and holiday gatherings.
