We Can Help

Hospice is a concept of care specializing in support for families and caregivers, and comfort for people with life limiting illnesses.  Hospice is for all age groups, including infants, children and adults.  Taking care of a loved one during the final stages of life can be very rewarding but also stressful.  Our role is to provide physical, emotional, social and spiritual support and bereavement services to the patient, caregivers, and family.

Hospice care is provided where it works best for you.  Most people prefer to stay at home, but care can also be provided in a nursing home, assisted living facility, hospital setting or hospice in-patient unit.  (Short breaks for the patient’s caregiver may also be available and arranged through hospice.

Visits will be scheduled on a regular basis.  The frequency of visits will depend on your specific need.  Staff is on duty and available 24 hours a day, 7 days a week, including holidays.

Your care team includes your doctor, the hospice medical director, pharmacist, nurses, hospice aides, social workers, chaplains, and trained volunteers.  We will work closely with you and your family/caregiver to develop a plan for your care.
